Peptide BlendsGROWTH HORMONE AXIS
Evidence grade D
Tesamorelin / Ipamorelin
The combination has never been studied, so it is graded D. On their own, tesamorelin is graded A (approved Egrifta products only) and ipamorelin C.
- Strengths
- 10 mg / 5 mg, 10 mg / 2 mg
- Contains
- Tesamorelin, Ipamorelin
- Molecule
- Blend of two peptides (10 mg is tesamorelin by inference from the COA)
- Mol. weight
- Tesamorelin 5,136 g/mol; ipamorelin 712 g/mol
Origin
A vial mixing tesamorelin, the GHRH analog approved as Egrifta to reduce excess abdominal fat in HIV-related lipodystrophy, with ipamorelin, an unapproved ghrelin mimic. The research vial is not Egrifta and has not been reviewed by FDA.
How it works
Tesamorelin acts on the GHRH receptor and ipamorelin on the ghrelin receptor, two separate pathways in growth hormone release research. Both are short pulse-type agents.
What the research shows
No study of tesamorelin plus ipamorelin was found. Tesamorelin's trials used it alone, and ipamorelin's human data are IV only. The mechanism rationale comes from older studies in which GHRH plus GHRP-6 released far more growth hormone together than alone, but those were different molecules.
Key studies
- Peñalva et al., J Clin Endocrinol Metab 1993 GHRH plus GHRP-6 (IV): GH area-under-curve 4,412 vs 821 and 973 alone.
- Bowers et al., J Clin Endocrinol Metab 1990 GHRP-6 acted synergistically with GHRH in normal men.
Safety findings
Tesamorelin's label anchors the safety picture: raised IGF-1 (above 2 SDS in 47%), glucose intolerance, swelling, carpal tunnel, joint pain and hypersensitivity, with contraindications in pregnancy, active cancer and pituitary disorders. Ipamorelin's trial showed more high blood sugar and low potassium than placebo, so blood sugar is the main stacked concern. Adding a second secretagogue may raise growth hormone and IGF-1 further, though this hasn't been measured.
Regulatory status
Tesamorelin is approved only as Egrifta WR/SV for HIV lipodystrophy and, as a biologic, can't be compounded. Ipamorelin is unapproved, lost its FDA advisory vote (0–12–1) in October 2024 and is on the 503B Category 2 list. No lawful compounded version of this blend exists, and WADA prohibits both at all times (S2).
Catalog notes
Available as 10/5 mg and 10/2 mg. The 10/5 mg lot TESIPA-0803 has a certificate (COA7626, August 17, 2026): 99.81% overall purity, with LC-MS/MS identity, endotoxin and heavy metals conforming; none is published for 10/2 mg. Check the lot's certificate for which figure is each component (it lists tesamorelin first); follow the storage stated on the label and certificate.
