Oxytocin

Grade A applies only to the FDA-approved hospital product (Pitocin and generic oxytocin) for labor and postpartum bleeding; nasal research on social and weight outcomes has been mostly null in well-run trials.

Strengths
2, 10 mg
Also known as
Pitocin (US brand), Syntocinon, Oxytocin acetate, α-hypophamine
Molecule
9-amino-acid neuropeptide hormone
Formula
C43H66N12O12S2
Mol. weight
1007.19 g/mol
CAS
50-56-6
Sequence
Cys-Tyr-Ile-Gln-Asn-Cys-Pro-Leu-Gly-NH2 (Cys1–Cys6 disulfide)

Origin

A natural hormone made in the brain and released from the posterior pituitary. It differs from vasopressin, the body's water-retaining hormone, at two positions.

How it works

Binds oxytocin receptors on uterine muscle and on cells around milk ducts, causing contractions and milk ejection. At large amounts it acts like vasopressin, increasing water reabsorption in the kidney. In animals it modulates social and stress circuits; how much nasal oxytocin reaches the human brain is unclear.

What the research shows

Decades of clinical use and trials support the approved injection for labor and postpartum bleeding, and it is on the WHO Essential Medicines list. Nasal research has been less consistent: the largest autism trial (290 children, NEJM 2021) found no benefit over placebo, and early positive findings on trust and social cognition have not held up in critical reviews. Small trials in obesity have not shown meaningful weight change. Animal work links it to bonding, maternal behavior, feeding and stress.

Key studies

  • Sikich et al., N Engl J Med 2021 (SOARS-B) 290 children with autism: no difference from placebo in social withdrawal at 24 weeks.
  • Nave, Camerer & McCullough, Perspect Psychol Sci 2015 Critical review: evidence that oxytocin increases trust in humans is not robust.
  • McCormack et al., J Endocr Soc 2023 Pilot in hypothalamic obesity: no meaningful weight change.

Safety findings

The approved label warns of water intoxication and low sodium with prolonged infusion, which has caused seizures, coma and maternal deaths, plus uterine rupture, arrhythmias and anaphylaxis. Nasal studies reported nasal irritation, headache and nausea at rates similar to placebo, and some pilot participants developed prolonged QTc. Approved products are labeled in USP units, not milligrams.

Regulatory status

FDA-approved as Pitocin and generic oxytocin injection for hospital obstetric use only (label revised March 2026); no approved nasal product was found, and approval does not cover research material. As a component of an approved drug it is 503A-eligible with a prescription and is not on the Category 2 safety-risk list. Not named on the 2026 WADA list.

Catalog notes

Available in 2 and 10 mg. No certificate or purity value is published yet; it is listed as oxytocin acetate with free-base formula and molecular weight, so check the lot's certificate for the salt form. Follow the storage stated on the label. Also offered as a liquid spray.
