P-21

Rodent data only, almost entirely from one laboratory; no human study has been published.

Strengths
5, 10 mg
Also known as
P021, CNTF-derived peptidomimetic
Molecule
4 amino acids plus one adamantylglycine (peptidomimetic)
Formula
C27H42N6O8
Mol. weight
578.70 g/mol
CAS
1246751-68-7
Sequence
Ac-Asp-Gly-Gly-Leu-(adamantylglycine)-NH2

Origin

Designed by Khalid Iqbal's group at the New York State Institute for Basic Research. It copies a short active piece of ciliary neurotrophic factor (CNTF), a natural brain-support protein, and adds an adamantane group to survive digestion and reach the brain.

How it works

Proposed from rodent work: it blocks leukemia inhibitory factor (LIF) signaling, raises brain-derived neurotrophic factor (BDNF) and phosphorylated CREB, and lowers activity of GSK-3β, an enzyme that adds phosphate groups to tau. Treated animals showed more new-neuron markers in the hippocampus and better synaptic protein levels.

What the research shows

Fed to mice and rats, P-21 increased new-neuron formation and improved memory in models of aging, Alzheimer's disease and Down syndrome. In a Down syndrome mouse model, treatment through the mother's diet rescued developmental delay and adult memory deficits; in female Alzheimer's-model mice, long-term treatment improved maze performance, though some synaptic results were only trends and groups were small. No human trial, pharmacokinetic study or case series exists, and the work has not been independently replicated outside the originating lab.

Key studies

  • Kazim et al., Scientific Reports 2017 Down syndrome mouse model: early treatment rescued developmental delay and adult memory deficits.
  • Baazaoui & Iqbal, Alzheimer's Res Ther 2017 Alzheimer's-model mice: better memory performance and more new-neuron markers after long-term treatment.

Safety findings

Side effects in people are unknown; there is no formal toxicology and no human safety data. Because it is designed to promote cell growth and survival, long-term cancer risk is an unstudied theoretical concern.

Regulatory status

Not FDA-approved for any use, and no IND or clinical trial was found. Not eligible for compounding (not on FDA's 503A nominations or safety-risk lists). Not named on the 2026 WADA list; as an unapproved substance it would fall under S0.

Catalog notes

Available in 5 and 10 mg. Two 5 mg certificates are listed for lot P21-5-0318 (99.8%, COA4866, June 17, 2026; 99.6%, COA3388, April 3, 2026), and none is published for 10 mg. Check the lot's certificate for the exact formula (a formula calculated from the sequence is C28H44N6O8); follow the storage stated on the label and certificate.
